Care Corner Cluster Support @ Toa Payoh provide quality, client-centric care for vulnerable seniors with limited family support and unable to cope with activities of daily living. We network with other service providers and local community partners to develop the necessary support network for seniors-in-need. This enhances the socio-emotional and healthcare needs of vulnerable elderly who may require assistance in activities of daily living and have limited family support.

By supporting the overall well-being of these seniors, we enable them to fulfil their dream of aging with grace and dignity in the neighbourhood of their youth.

A key component of the Cluster Support programme is the Caring Assistance from Neighbours (‘CAN’) programme. Here, neighbours are encouraged and empowered to take on the role of a caregiver to provide close monitoring and look out for the well-being of the frail and vulnerable elderly during home visits. Such caregivers may supervise seniors in packing of medicine, remind these seniors to take their medication, monitor their health, and ensure that the seniors attend their required medical appointments. The caregivers will have regular meetings with the case workers to update and review the senior’s progress.

Care Corner CREST @ Toa Payoh was launched in Toa Payoh estate to facilitate better integration of seniors with mental illness and their families with the community, health and social agencies and the government. It also serves to aid the caregivers who may be susceptible or suffering from caregiver burden (e.g. stress, anxiety, and depression).

This programme promotes awareness and early recognition of signs of depression and/or dementia. In addition to early diagnosis and intervention, the service focuses on education of the seniors and their caregivers on managing dementia and the mental illness and equip them with coping strategies, provision of emotional support, and locating the right care and assistance within the healthcare and social services systems e.g. hospitals and clinics for medical and psychiatric care; welfare groups and community service organisations for rehabilitation, social and financial assistance.
